Vendors should provide the following information with their form: proof of current liability insurance, proof of valid commercial pesticide applicator's license from the state of Alabama; and at least 3 professional references that can answer questions about similar work you have performed.
Remember that this program is for a duration of 3 years and will drastically ramp up in 2011. If you are not selected in 2010, just send us any updates on the required documentation.
