Currently we are requesting that any vendors interested in participating in the program to download and submit the following form:  Vendor Request Form (pdf).  You need Adobe Acrobat to view the file; if you don't have it you can download the free version here. Vendors should provide the following information with their form: proof of current liability insurance, proof of valid commercial pesticide applicator's license from the state of Alabama; and at least 3 professional references that can answer questions about similar work you have performed.

Remember that this program is for a duration of 3 years and will drastically ramp up in 2011. NOTE TO THOSE DOWNLOADING THE FORM: If you have Adobe Acrobat Reader (the free version of Adobe Acrobat) this form will only print; you cannot save it. Just complete and print a copy for your records and fax or mail to us. Also, the "Submit by Email" button works but only if you use Outlook, Outlook Express, Thunderbird, etc. as your email client. The email functionality does not currently work if you use AOL or a web browser (Internet Explorer, Chrome, Firefox, etc.).
